Authorization for Medical Care

If if becomes necessary for my child to have medical care while participating in this program, I. a parent or/and legal guardian, herby give I-Love-School personnel permission to use their judgment in obtaining medical care for the child, and I, a parent or/and a legal guardian, given permission to the physician selected by I-Love-School personnel to render medical care deemed necessary and appropriate by the physician.
It is understood that the resulting expenses will be the responsibility of the student. 

Agreement of Liability Release

I understand that I hold the I-Love-School or/and Virgil Middle School and its agent and employees harmless from any and all liabilities or financial claims which may arise out of connection with the student’ participation in this program or/and activities, including transportation to and from the pick up point.
